Family Intervention Team (FITT) is an intensive in-home, community based program supporting youth ages 5-18 who are at risk of hospitalization, disruption or needing stabilization support when returning home from a higher level of care. FITT partners with families to avoid out-of-home placement and provides skills for future crisis prevention.

How We Do It
Collaborate
We work with the family to create a comprehensive plan of care and collaborate with community partners, DCS, schools, juvenile court, etc.
Engage
We provide wraparound services of support and assist families in accessing needed community resources, including crisis stabilization. Services are scheduled to match family availability within the home, community or school.
Support
Families receive parenting support and needed connections to community resources. Team members are available 24/7 for crisis support. Expedited medication services are provided so youth can see a provider within one week or even days to have their needs met. Providers are skilled in educating caregivers on safety and can conduct safety sweeps of the youth or home environment.
TBRI
Providers are skilled in educating caregivers on safety and can conduct safety sweeps of the youth or home environment. FITT follows a Trust-based Relational Intervention model that focuses on the family system. It is an evidence-based, trauma-informed intervention that is specifically designed for children who come from hard places, such as maltreatment, abuse, neglect, multiple home placements, and violence, but you'll see that the principles apply to all children. TBRI® consists of three principles: Connecting, Empowering, and Correcting which have been used in homes, schools, residential facilities, and several other settings. They are founded in research and designed for use with children and youth of all ages and all risk levels.

Each family in our 90-day program receives evidence-based services from a Master-level Therapist and a Family Support Specialist dedicated to their care with 24/7 access to on-call providers.
